AZ Kid Clubs makes life easy for our parents. Drop off & Pick up is a simple sign-in / sign-out sheet with the time and your signature.

Below is a printable copy of the AZ Kid Clubs Immunization Form and Contract. We need specific information regarding each new child and parent for our record keeping. These documents are required on the day of, or before your starting date.

About our Payment Schedule

Full Time  is due by the start of the week on Monday @ 7:00am

Part Time  is due by the end of the week before Friday @ 5:30pm, or the start of the next week by Monday @ 7:00am
